Knee ACL Injuries: When Can I Return to Sport?

An ACL (anterior cruciate ligament) injury can be a significant setback for athletes and active individuals. Whether you’re a weekend warrior or a competitive athlete, one of the most pressing questions after an injury is: “When can I return to sport?” The answer is multifaceted, relying not just on time but also on meeting specific rehabilitation milestones.

Understanding the ACL

The ACL is a crucial ligament that stabilizes your knee, especially during activities involving sudden stops, changes in direction, or jumping—common in sports like soccer, netball, basketball, and skiing. This is why the ACL is vulnerable and can be injured during these sports and movements.

The Rehabilitation Journey

Post-injury treatment often involves surgical reconstruction, though some individuals may opt for non-surgical management depending on their activity levels and goals. Regardless of the treatment path, a structured rehabilitation program is essential to restore knee function, strength, and confidence.

When Can I Return to Sport?

While it’s tempting to set a fixed timeline—such as six months post-surgery—research indicates that returning to sport should be based on specific criteria rather than time alone. Premature return can increase the risk of re-injury.

Key Criteria for Return to Sport:

  • Strength Symmetry: Achieving at least 90% limb symmetry index (LSI) in quadriceps strength compared to the uninjured leg is crucial. This is often assessed using isokinetic testing.
  • Functional Performance: Successfully completing a series of hop tests, including single-leg hop for distance, triple hop, and crossover hop, with at least 90% LSI.
  • Movement Quality: Demonstrating proper biomechanics during dynamic activities such as landing, cutting, and deceleration tasks.
  • Psychological Readiness: Assessing confidence levels using validated tools like the ACL-Return to Sport after Injury (ACL-RSI) scale to ensure the athlete is mentally prepared to return.

Typical Timeframes (Individual Variability Applies)

  • Low-Impact Activities (e.g., cycling, swimming): 3–4 months
  • Running: Approximately 4–6 months
  • Non-Contact Sport Drills: 6–9 months
  • Full Contact Sport: 9–12 months, provided all criteria are met

It’s important to note that returning to sport before nine months post-surgery is associated with a higher risk of re-injury. Athletes who returned before this timeframe were found to have a sevenfold increased risk of a second ACL injury.
