The Truth About Sciatica and How Physiotherapy Can Help

Sciatica is one of the most misunderstood causes of lower back and leg pain. Whilst many people use the term broadly, true sciatica refers to irritation or compression of the sciatic nerve – the largest nerve in the body, running from the lower spine down into each leg.

Common symptoms include sharp or burning pain radiating down one leg, tingling, numbness, or weakness. These symptoms are often triggered by conditions such as disc bulges, spinal narrowing, or muscle tightness. Not all leg pain is sciatica, which is why proper assessment is important.

Low back pain is incredibly common and closely linked to sciatica:

• Around 619 million people worldwide were living with low back pain in 2020

• It affects 7–8% of the global population at any time

• Cases are projected to rise to over 840 million by 2050

A significant portion of these cases involve nerve-related symptoms like sciatica, making it a widespread issue that impacts daily function and quality of life.

Common Myths

• Bed rest is best: In fact, prolonged rest can delay recovery. Gentle movement is usually more helpful.

• It will go away on its own: Some cases improve, but others may persist without treatment.

• “Surgery is inevitable”: Most people recover with conservative care, including physiotherapy, before surgical review is needed.

How Physiotherapy Helps Sciatica

Physiotherapy for sciatica addresses both the symptoms and the underlying cause. Treatment may include:

• Techniques such as manual therapy and guided exercises to provide symptom relief and reduce nerve irritation or compression

• Strength and mobility programs to support the spine and improve movement

• Posture education and activity modification to reduce strain in everyday activities

• Graduated return to activity to safely rebuild confidence and function

Early physiotherapy can reduce pain, speed up recovery, and lower the risk of recurrence.

When to Seek Help

It is crucial to seek prompt medical care if symptoms are severe or progress towardssignificant weakness or changes in bladder or bowel function. Otherwise, early physiotherapy is highly effective in managing most cases.
